The online Form B is mandatory by InfoSpec. Before you present please make sure you have read and understood the AIA Quality Commitment Statement. The AIA requires all providers to have one on file for each presenter.

We need a signed copy in our files before you present. After you provide us with your copy you will receive a username and password to submit Form Bs.

Presenters please select your name from the presenter's drop down list. If your name is not in the list you have not signed and faxed your signed AIA Quality Commitment Statement to InfoSpec. (You can also email the Statement to carol@ronblank.com) Please check the CD-ROM provided by InfoSpec it contains a copy of the AIA Statement.
